The services range from home aftercare following diagnosis, psychosocial support in family emergencies, to training of educational professionals in daycare centers and schools. Through the nationwide network of more than 200 Dianiño Diabetes Nannies, it is ensured that affected children and adolescents receive security and guidance in their daily lives and can actively participate in daycare and school activities.
Eligible for funding are public institutions that submit an informal assistance request via the completed inquiry form – submitted by treating physicians or diabetes teams. Besides proof of type 1 diabetes diagnosis and residence in Germany, no further documents are required. Requests can be submitted online at any time; there is no application deadline. With this program, the foundation aims to sustainably strengthen the independence and quality of life of young people with type 1 diabetes through needs-based, professionally coordinated care and thus effectively relieve families during challenging phases.
