Description
The funding program supports Hessian municipal administrations in establishing positions for community care coordinators who act as an interface between medical and nursing care in rural areas. With a funding rate of up to 80% of eligible personnel costs and a maximum amount of €50,000 per year, qualified geriatric nurses, social medical assistants, medical assistants with VERAH qualification, or social workers are supported. The goal is to identify the care needs of elderly and dependent individuals early, develop individual support plans, initiate preventive services, and close local care gaps. Public-law institutions such as districts, independent cities, and district-affiliated municipalities with the endorsement of the responsible district are eligible to apply.
Funding requires the submission of a valid district-wide or city-specific concept explaining the planned fields of activity and network collaboration. The position must cover at least 50% of the collectively agreed weekly working hours, and overall financing must be secured. The application must include proof of the community care coordinators’ qualifications, the financing concept, and the district-wide concept. Applications can be submitted to the Hessian Ministry for Family, Seniors, Sports, Health, and Care by February 28, June 30, and October 31 each year. The program contributes to sustainably strengthening care structures in rural areas and ensuring comprehensive, high-quality care provision.
