Funding objective

Implementation of a nationwide hospice culture and palliative care in mobile nursing and support services with the following objectives: recognition and support of clients in their final phase of life; enabling dying at home; promotion and expansion of regional structures; implementation of anticipatory care plans; competent support for relatives; professional development and relief of staff; improvement of communication and collaboration within the organization and with system partners.

Funding requirements

  • The organizational and quality development process is conducted in conjunction with several "HPC Mobile" workshops for at least 80% of employees across professional groups and organizational units.
  • Appointment of two palliative care officers and a palliative care group (from 30 employees) within the organization, who jointly develop work assignments with process facilitators.

Description

HPC Mobil is initiating a 30-month organizational development process in Tyrol to sustainably strengthen the hospice culture and palliative care in mobile nursing and care services. Through several interdisciplinary and cross-organizational workshops, proactive care plans and regional networking structures will be established. The focus is on perceiving and supporting clients in their final phase of life as well as enabling a dignified death in the home environment. In parallel, two palliative care officers and a palliative care group will be appointed, who, together with experienced process facilitators, will identify and implement work assignments. The professional development of employees is to be continuously promoted, psychological and physical burdens reduced, and collaboration with system partners intensified.

Eligible for funding are non-profit organizations, companies, and public institutions in Tyrol that offer mobile nursing and care services. Applications can be submitted annually between January 1 and December 31 of a given year, as long as the funding volume has not been exhausted. The goal is a nationwide implementation of a hospice and palliative care culture in Austria that strengthens the support of relatives as well as interdisciplinary cooperation within the organization. The grant enables organizational leadership and teams to be supported and relieved in their quality development, thereby ensuring holistic end-of-life care and sustainably expanding regional networks.
