Funding objective

Description

Short-term transitional care in Vienna provides temporary inpatient support for individuals in need of care following a hospital stay or serious illness. This funding offer grants subsidies for care, accommodation, as well as therapy and remobilization services in nursing homes in accordance with the Vienna Residential and Nursing Home Act for up to 92 days. The prerequisites are a care requirement from care allowance level 2 or a social indication, a primary residence or temporary stay in Vienna, and the application submission at the customer service of the Fonds Soziales Wien before admission or within a few days after hospital discharge. The funding is provided as an ongoing subsidy and is based on the actual costs incurred for accommodation and care. A negative decision cannot be derived, as each decision is based on an individual professional assessment and funds can be applied for at any time.

The application must include a current medical report, registration certificate, proof of care allowance level, as well as documents regarding citizenship or equivalence. If necessary, the FSW collects additional information on income and rental costs to calculate the cost contribution; this is charged for corresponding incomes. Criteria such as medical and nursing necessity, availability of suitable places, and economic need are considered in the decision. Upon approval, the payment is made directly to the facility. Changes in care needs or financial situation must be reported to the fund immediately.
