About the funder

The non-profit foundation Aktion Wandlungswelten is a legally recognized foundation under civil law based in Jena, Thuringia. It was established in April 2003 and has been a member of the Paritätischer Wohlfahrtsverband since November 2004. With an endowment of €50,000, it supports independent non-profit organizations dedicated to the care and integration of sick, socially disadvantaged, mentally ill, disabled, or at-risk children, adolescents, and adults. Mission & Vision

The foundation is committed to the equal participation of sick and disabled people in society. It selflessly supports organizations that provide medical, social, educational, psychological, and cultural care to these target groups and strives for a self-determined life in accepted living environments. By supporting integration, cultural, community, and research projects, the foundation aims to promote responsibility and enable the self-realization of the affected individuals.

Target groups

Funding is provided to independent non-profit organizations that care for sick, socially disadvantaged, mentally ill, and disabled or at-risk children, adolescents, and adults. This includes providers of sheltered housing, rehabilitative facilities, employment initiatives, cultural activity projects, as well as outpatient, day-care, and inpatient services within the framework of the Social Code (SGB), the Thuringian School Act, and the Vocational Training Act.
