Funding objective
The purpose of this funding is to provide financial support to individuals in need of care and assistance immediately following a hospital stay for qualified short-term care/qualified follow-up care (transitional care) in a specialized care facility, with the goal of enabling the care-dependent person to return to home care and live independently at home.

- Medical confirmation of the necessity for qualified short-term care/qualified follow-up care

- Income statements of the person in need of care or support and, if applicable, their partner
- Proof of maintenance claims or obligations
- Bank statements of the last 6 months of the person in need of care or support and, if applicable, their partner

- Medical certificate confirming the necessity of qualified short-term care/qualified aftercare and assessment regarding return to home care
Description
The “Promotion of Qualified Short-Term Care (Transitional Care)” by the State of Tyrol is aimed at care-dependent and support-needing persons with Austrian citizenship or equivalent status, who receive care allowance levels 1–7 or have submitted a corresponding application. The goal of this ongoing subsidy measure is to enable a seamless transition into a specialized care facility immediately following a hospital stay, thereby promoting the return to home care and independent living. Applications can be submitted within six months after the start of the qualified short-term care. The support is exclusively limited to facilities in Tyrol and is granted as a one-time subsidy.
Funding Requirements and Documents
Eligible are persons with main residence in Tyrol who are not expected to move into permanent inpatient care and can provide a medical certificate confirming the necessity of qualified short-term care. For the application, among other documents, proof of the duration of care utilization, current income and bank statements, a registration certificate, as well as documents regarding maintenance claims are required. The complete documents must be submitted to the Office of the Tyrolean State Government, Department of Care. Careful preparation ensures prompt processing and guarantees that those affected can quickly benefit from financial relief during the transitional phase.
