Description

In Styria, an innovative pilot project for transitional care is being funded at the Mürzzuschlag Regional Care Center, supported by grants from the State of Styria until 2026. The goal is to establish a specialized care unit bridging the gap between acute hospital stays and full recovery, thereby closing the aftercare gap. The initiative targets patients with prolonged convalescence who no longer require intensive hospital services but are not yet strong enough to return to everyday home life or a remobilization unit. As a complement to existing services, the project creates a needs-based environment where medical professionals and nursing experts support rehabilitation, ensuring a smooth transition.

Eligible for funding are public institutions in the health as well as labor and social sectors, which may apply for a grant. Applications can be submitted continuously from November 12, 2025, with no fixed deadline. A key prerequisite for funding is proof of bed days occupied in 2025. This offer particularly benefits adults in the fields of internal medicine, neurology, surgery, and orthopedics/traumatology when internal medical conditions, functional impairments of the musculoskeletal system, major surgeries, or chronic therapy-resistant conditions require ongoing care. Public providers are thus enabled to implement a needs-based care model that sustainably enhances patients’ quality of life and optimally meets regional care demands.
