Funding objective
Support in the development of outpatient, inpatient, and cross-sector concepts that ensure, further develop, and innovatively design basic medical and nursing care for the population throughout Schleswig-Holstein.

Funding requirements
- Implementation of the project in Schleswig-Holstein
- Model must improve quality of care and/or efficiency of care
- Concept must also be transferable as a model to other care regions
- Project must be consistent with medical needs planning and, if applicable, hospital planning
- No disproportionate impairment of other contract physicians (competition neutrality)

Description
The state of Schleswig-Holstein supports the development of innovative concepts and construction measures to secure and qualitatively further develop outpatient, inpatient, and cross-sector medical primary care through grants. Eligible for funding are, among others, outpatient contract physicians, medical facilities, recognized physician networks, hospital operators, rehabilitation and care facilities, as well as municipalities, districts, offices, and special-purpose associations. The goal is to create demand-oriented care models that not only increase care quality and efficiency but are also transferable to other regions and align with medical needs and hospital planning. The principle of competitive neutrality applies to avoid disproportionate disadvantages for established contract physicians.
Funding is provided, among other things, for the conceptual development of interdisciplinary care models, mobile, technical, and digital solutions for patient care, as well as investments in cross-sector innovations, such as the conversion of small hospitals into primary care centers or medical family centers. Construction measures for the initial provision of new services are also eligible. Eligible expenses range from a minimum amount of €25,000 up to a maximum of €500,000 per project, with a duration of up to 36 months. Pure basic research, routine standard care without innovation potential, and clinical studies to demonstrate the efficacy of pharmaceuticals and medical devices are not funded. Currently, no applications are being accepted – interested parties will be informed as soon as the application period reopens.
