Description

Social psychiatric individual support/case management in Tyrol offers adults and adolescents from the age of 16 with mental illnesses and significant impairments of their psychosocial abilities hourly outpatient support services for self-determined living and the strengthening of social participation. The goal is to improve and maintain mental health, quality of life, and relational capacity through individual psychosocial support, sociotherapy, social counseling, crisis intervention and prevention, as well as motivation in the areas of health promotion, daily structuring, leisure activities, and relationship building. The service is provided flexibly in the home environment, at service providers’ facilities, or during external activities and is granted within the framework of the Tyrolean Participation Act for a maximum of 60 months. The funding is based on the principles of empowerment, participation, recovery, and self-help and requires low-threshold access.

Eligible for funding are private individuals residing in Tyrol with a disability recognized according to § 3 lit. a TTHG, Austrian citizenship or equivalence, and the prospect of sustainable strengthening of participation. Applications can be submitted year-round; services can be granted from the month of receipt of the complete application. For approval, proof of disability and the completed application form according to TTHG are required. The Department of Inclusion and Child and Youth Welfare of the State of Tyrol reviews the applications taking into account individual needs and resources. Social psychiatric case management ensures that people with mental illnesses are supported and relieved in all phases of their life design to achieve lasting independence and participation.
