Housing and Care - Short-term Care Remobilization

Short-term care remobilization is aimed at clients in Vienna who, following a hospital stay, have an increased need for medical-therapeutic care and require temporary accommodation in a recognized facility due to their care and support needs. Subsidies for accommodation costs, nursing services, and therapeutic offerings are granted for a maximum period of 92 days to enable rapid remobilization and the recovery of the highest possible level of independence. The ongoing funding option ensures a straightforward application process at the client service of the Fonds Soziales Wien (FSW), allowing care to seamlessly continue following the hospital stay.

Eligible for funding are private individuals with their main residence in Vienna who cannot cover their increased medical and therapeutic care needs through mobile services and who have a legal entitlement to the benefit. Applications can be submitted at any time, with clients required to submit the following documents: the completed FSW application form, a medical report (no older than six months), the registration certificate, proof of receiving care allowance, and proof of residence in Vienna. The amount and duration of the subsidy depend on the individual care and support needs as well as the financial circumstances. This targeted funding enables professional, time-limited short-term care that focuses on stabilization and mobilization and supports a sustainable return to the familiar everyday environment.
