Description

Transitional care in Carinthia offers temporary support for individuals in need of care following a hospital stay and serves as a bridge until their return to the familiar home environment. Public institutions, non-profit organizations, and companies with a valid permit under the Carinthian Home Act may apply for a subsidy for nursing and care services. Funding is provided for up to 28 days, and in exceptional cases, up to 42 days. The objective is to ensure continuity of care by guaranteeing a seamless transition from acute treatment to inpatient or alternative living arrangements in accordance with § 16 para. 2 lit. a K-HG. The funding falls within the thematic areas of social affairs, health, as well as labor and social matters and strengthens the regional care infrastructure in Carinthia.

Prerequisites for the application include proof of compliance with the principles of transitional care according to §§ 1–3 of the guideline and a permit under the Carinthian Home Act. The application procedure requires a completed funding application, confirmation that the person in need of care cannot yet be cared for in a home environment, as well as an indication list or comparable documents from discharge management. Applications have been accepted continuously since 01.01.2014 – there is no time limitation. The daily co-payment amounts to 1/30 of the care allowance and is collected by the respective operator. This funding instrument makes a valuable contribution to strengthening the care infrastructure and relieves the healthcare system through demand-oriented bridging care for the period following hospital treatment.
